Remote monitoring is a way for our medical team to monitor you in your home environment. It uses Bluetooth
connected medical devices and wearable devices such as Fitbit to beam this information to a secure server where
our team can review it. By using the BioAnalytica app, you have a secure platform of communication with our
support team. This advanced phone app allows monitoring of your basic vital signs, level of activity, physiologic
response to activity and even nutrition.
Whether you are unable to meet the travel requirements and time commitment of a traditional hospital based
program or have just completed an inpatient or outpatient rehabilitation program, we can customize a
rehabilitation program for you.

Setting Expectations
​
Due to time and travel requirements and limited availability, hospital based rehabilitation is not available to
everyone. Remote wellness and rehabilitation allows an individual to undergo the education and activity
monitoring of a rehabilitation program in the comfort of their own home.
Remote monitoring wellness and rehabilitation does not take the place of a well-designed hospital based
pulmonary rehabilitation program staffed by professional staff. However, we are able to look at your vital signs
(heart rate, activity, blood pressure) remotely. We evaluate these parameters over several days and weeks as you
progress through a program and are there to provide guidance and support.
Our program is customized, every patient receives an individualized plan, including exercise and education. Our
team works with you to build a treatment plan that will ensure you to get on the road towards improving your
health.

A Comprehensive, Tailored Program
​
Your individualized plan begins with a telemedicine consultation with our Medical Director, who evaluates your
health condition, medical history and current level of conditioning into account to develop exercise guidelines
tailored specifically for you.
Our caring staff remotely monitors your daily activity, walking speed, oxygen level and blood pressure – providing
feedback, encouragement while building confidence as you progress through your program.
We complement the exercise with thoughtful education and guidance on ways to increase your endurance for daily
activities, breathing techniques, management of respiratory symptoms, exercise, stress management and nutrition.
We also provide educational videos on helpful topics, such as understanding lung disease, COPD, oxygen use,
parameters for safe exercise, and smoking cessation.

Pulmonary Rehabilitation

Most patients with pulmonary disease can maintain or improve their usual level of activities and quality of life if
they learn how to safely live with their condition and develop healthy habits and routines. Although not a replacement for in person hospital based programs, the MiHealth program allows pulmonary rehabilitation in the comfort of your own home.

Program Details
• Our comprehensive, tailored six week program begins with an initial phone call to evaluate your goals, physical limitations and self-motivation to make sure that a remote rehabilitation program is right for you.
• You will then enroll online through our secure web interface and download of the BioAnalytica app on your phone or tablet.
• All participants will be required to purchase their own monitoring equipment including a Fitbit ($99), an iHealth Bluetooth pulse oximeter ($79) and iHealth blood pressure device ($35) ( best prices through Amazon online). These equipment pair directly with the MiHealth/BioAnalytica app, allowing our staff to monitor your daily activity.
• Since not everyone is a techno-wiz, our staff will contact you regarding the technical aspects of connecting the devices to your app.
• Once enrolled and set up, we will schedule a telehealth consultation by our Medical Director/Pulmonologist. As part of the initial evaluation, you will be asked to fill out a basic questionnaire regarding your current health status. You will also perform a 6 minute walk test at home using your Fitbit and pulse oximeter. We utilize this important information as we develop your individualized rehabilitation plan.
• Our caring staff monitors your daily activity and vital signs and provides confidence-building encouragement as you exercise, with the goal of your steady, safe progress.
• We complement the exercise with thoughtful guidance on ways to increase your endurance for the daily activities that matter to you through educational videos on breathing techniques, management of respiratory symptoms, exercise, stress management, and nutrition sent directly to your MiHealth BioAnalytica app.
• Our staff will also provide you with extensive education on helpful topics, such as understanding lung disease, COPD, oxygen use, parameters for safe exercise, and smoking cessation.
